Organizers include the Academy of Practice and Science (APW) of the German Society of Dentistry, Oral and Maxillofacial Surgery (DGZMK) and the training institutes of the Hamburg, Schleswig-Holstein, Lower Saxony and Westphalia-Lippe dental associations.

The practical intensive courses usually take place on the premises of the CMD Center Hamburg-Eppendorf due to the optimal conditions on site and the convenient transport connections. In addition to digitally networked treatment rooms, a well-equipped seminar room and reception area are available here. The in-house bistro provides refreshments. An espresso at the end of lunch quickly gives us constructive ideas again…

Functional diagnostics and therapy for my practice?

Introductory seminar for orientation for interested parties

Current representative studies show that around 10% of patients in dental practices suffer from craniomandibular dysfunctions (CMD) that require treatment. This means that every dentist in his practice has patients with craniomandibular dysfunction (CMD) requiring treatment. The incidence of patients with bruxism is even higher. Not recognizing those affected can end just as unpleasantly for the patients as for the dentists.

Dentists are therefore faced with the challenge of recognizing functionally conspicuous patients in order to decide to what extent these patients should be treated in their own practice and when it makes sense to refer them for co-treatment.

In view of the limited university training in this area and the complexity of the procedures to be used as well as the patients and clinical pictures, this decision will be different for each practice.

A clearly structured concept is required to identify the patients concerned and to decide on the course of treatment. The seminar presents this as well as the necessary examination steps, concepts and documentation tools and thus provides a guideline for the individual orientation of your own practice.

Curriculum function, CMD and bruxism

Structured training from a single source in Hamburg

The dentistry curriculum in Germany provides only little teaching in the field of diagnosis and treatment of craniomandibular dysfunctions (CMD) and bruxism; the new licensing regulations further curtail the topics.

To compensate for this, the Hamburg and Schleswig-Holstein dental associations jointly offer the Hamburg Curriculum Function, CMD and Bruxism. The scientific director is Priv.-Doz. Dr. Ahlers.

This curriculum was developed in 2008 from a series of CMD courses. This was expanded into a curriculum and has been offered and updated regularly since 2009; the ratings are consistently first-class. Exactly one curriculum starts each year with a maximum of 20 participants. In the intensive practical exercises, the cohort is divided into 4 groups of 5 participants each. The curriculum takes place on the premises of the CMD Center Hamburg-Eppendorf and is regularly fully booked. Ms. Wegemann from the Hamburg Dental Association is already accepting registrations for the next curriculum, which starts at the beginning of next year.

Duration: The curriculum comprises 10 dates spread over a total of 18 months. All courses take place in the training rooms of the CMD Center Hamburg-Eppendorf; the final event takes place at Dorint-Hotel Hamburg-Eppendorf.

Online course series Bruxism, CMD + Restorations

Bruxism can lead to CMD and tooth wear. The level of suffering for those affected is sometimes very high. Treatment should be as individualized as possible and depends on the initial situation. However, this first requires sufficient diagnostics to determine the proportion of the various problems of bruxism, CMD and the extent of tooth wear.

As face-to-face training can only take place to a limited extent at present due to the pandemic, PD Dr. Ahlers is offering a structured series of courses on the diagnosis of bruxism, CMD and the restorative treatment of resulting consequential damage via the organizer IFG. The course series comprises 8 courses and has been very well received. This is helped by the fact that you can view the individual courses at any time and repeat them as you wish. Interested? Online course Abrasion dentition: Diagnostics + therapy

Tooth wear is an increasing problem. The incidence is rising rapidly, especially among young patients, and in some cases reaches 25% in Dutch studies. This means that a quarter of the young people affected have damage to the dental hard tissues that requires treatment. It is therefore important for dental practices to recognize these patients at an early stage and to identify the individual causes in those affected. To this end, a Dutch-German working group has developed a multi-stage diagnostic procedure, the Tooth Wear Ealuation System, TWES. This was initially published at the University of Amsterdam ACTA and further developed in cooperation with colleagues from the CMD Center Hamburg-Eppendorf.

In practice, the procedure is based on a two-stage diagnostic concept.

  • The first step is a tooth wear screening to identify conspicuous patients.
  • In the second stage, these are examined in more detail (per tooth) with a correspondingly more intensive tooth wear status, supplemented by an assessment of the pathology and the recording of findings that allow conclusions to be drawn about the aetiology. Individual treatment planning is then carried out on this basis.

Both examination steps are now recognized by the German Dental Association as independent services. To implement the procedure in the dental practice, corresponding software is now available to expand the practice software, CMDbrux, the use and application of which is also taught in the course, so that the implementation of the knowledge in the practice is immediately guaranteed.

Ceramic veneers: restorative – functional – esthetic

Today, treatment with ceramic veneers is a scientifically recognized definitive type of restoration (scientific communication of the DGZMK and DGZ). In addition to esthetic indications, ceramic veneers are now also suitable for the restorative treatment of anterior tooth defects and for permanent functional correction with restoration of canine-protected occlusion (“canine guidance”).

This requires special preparation forms that correspond to the indication and the characteristics of the materials used and enable safe further clinical treatment with predictable results.

In practice, three different types of preparation have emerged. These include classic (labial) veneers, which primarily bring about aesthetic changes and in which the surface on the lip side is only minimally abraded. In the case of severe discoloration and, in particular, extensive substance deficits, newer forms of treatment are used (“medium wrap” and “full wrap” veneers). Controlled long-term studies are now available on the various modifications, which confirm their durability.

The prerequisites for programmed treatment success are, on the one hand, carefully structured esthetic and functional treatment planning, and a preparation technique that predictably limits the depth of penetration to the enamel during preparation – this contributes to a 10x higher life expectancy of the veneers.

The training course systematically describes the various indications, explains the findings to be collected and the material requirements. On this basis, the preparation forms for the various ceramic veneers are explained step by step. This is followed by a brief overview of the dental fabrication options and adhesive cementation.

Event format: Lecture or practical intensive course

(In practical intensive courses, the various treatment steps are then practiced on the simulator. After a demonstration by the course instructor, Priv.-Doz. Dr. M. Oliver Ahlers, the course participants first carry out preparation exercises using preparation instruments developed by the course instructor). This can be followed by training in the fabrication of temporary restorations after the preparation of extended veneers.

Working group “CMD + pain”

History: The “Chronic Pain / CMD” working group of the Hamburg Dental Association was originally set up on the initiative of the then training officer of the Hamburg Dental Association and the Medical Director of the Northwest German Maxillofacial Clinic (NWDKK) at the UKE, Prof. Dr. Dr. Pfeifer.

It was initially headed by Prof. Dr. Dr. Karsten Gundlach, then senior physician under Prof. Pfeifer. After his appointment to the University of Rostock in 1992, Prof. Gundlach transferred the management of the working group to Priv.-Doz. Dr. Ahlers. A change took place in 2007: since then, the chamber’s working group has also formed the Hamburg study group of the German Society for Functional Diagnostics and Therapy (DGFDT).

Activities: The focus of the working group has always been on organizing collegial meetings on the premises of the UKE, now the CMD Center Hamburg-Eppendorf. Members discussed the latest literature or invited specialists on selected topics. Another focus is the exchange of information on the most effective treatment possible for patients who have already been diagnosed.

Meetings: The meetings last about 3 hours and take place in the seminar area of the CMD Center Hamburg-Eppendorf, in the building of the Center for Innovative Medicine (CiM) at Falkenried 88, see Contact.
